Durable Power of Attorney: A power of attorney grants another person the right to act on your behalf, as if you were present and acting, with respect to your assets and your property rights. The power you grant can be limited or broad and can be contingent upon the happening of a specified event, such as incapacitation.

Health Care Surrogate: A health care surrogate names someone to act if you become unable to make medical decisions yourself.

Living Will: A living will is a form that states your wishes regarding the use of life-sustaining measures if you become terminally ill. A living will works with the Designation of Health Care Surrogate to provide a full set of directions to your physician.
