The power you grant can be limited or broad and can be contingent upon the happening of a specified event, such as incapacitation.
Health Care Surrogate: A health care surrogate names someone to act if you become unable to make medical decisions yourself.
Living Will: A living will is a form that states your wishes regarding the use of life-sustaining measures if you become terminally ill. A living will works with the Designation of Health Care Surrogate to provide a full set of directions to your physician.
